What information will you obtain from me?

We collect information called Personally Identifiable Information (PII) in order to accurately complete your application for health insurance. PII can be used to identify you and your identity.

Will you share my information?

We may only share your information as described in this notice; that is, with certain Federal or State agencies, the health insurance company you select, or subcontractors that allow us to provide services to you.

We must be given your consent and permission to share any information described in this notice.

What happens if I don't share my information?

If you elect to not share information, you may not be able to enroll in a health insurance plan.

Is my information safe?

Yes. We are required to protect your information appropriately. We take adequate measures to insure your PII is handled with integrity and care.
